Department: CSR Behavioral Health Services
Reports to: Community Support Program Coordinator
FLSA Status: Exempt

Program Description:

The Community Support Programs (CSPs) provides an array of services delivered by a community-based, mobile, multidisciplinary team of paraprofessionals. These programs provide essential services to individuals with a long-standing history of a psychiatric or substance abuse disorder and to their families, or to individuals who are at varying degrees of increased medical risk, or to children/adolescents who have behavioral health issues challenging their optimal level of functioning in the home/community setting.

Essential Duties and Responsibilities:

  • Assists clients in enhancing their daily living skills;
  • Provides service coordination and linkage;
  • Encourages and facilitates the utilization of peer support, self-help groups and natural support systems;
  • Provides outreach, care management monitoring, follow-up, and general assistance for clients in dealing with day-to-day activities or problems that may impede access to treatment or the progress of recovery.
  • Assists clients with: obtaining benefits, housing, and health care; with transportation to appointments and other activities;
  • Provides clients and their families with education/training, educational materials about mental illness, substance abuse, recovery and rehabilitation.
  • Trains and provides assistance in decision-making/problem solving, vocational guidance, skills building, and supports in both crisis and non-crisis situations;
  • Facilitates access to education and training on the effects of psychotropic medications, as well as those for physical disorders
  • Work collaboratively with state agencies and other providers
  • Facilitates and serves as an adjunct to psychotherapy services and primary care services for medical issues.
  • Ensures that clients are linked to ongoing medication-monitoring services and regular health maintenance.
  • Completes all required paperwork according to guidelines and on a timely manner
  • Other duties may be assigned.


Educational/Experience Requirements:

- Must have a Bachelor’s degree in human services field from an accredited university and one (1) year of experience working with the target population, or

- Paraprofessionals capable of meeting community support needs of psychiatric issues for children, adolescents, and adults, as well as issues related to substance use and/or co-occurring disorders

- Specialized training in child/adolescent development (for child/adolescent CSP programs), behavioral treatment, substance abuse/dual recovery issues, and family treatment/engagement/education regarding psychiatric and substance abuse recovery issues

- Ability to reason effectively and interpret a variety of instructions, written or oral

- Strong writing and interpersonal communication skills

  • Operate well in a fast paced environment and as part of a team
  • Computer skills using Windows based software
  • Treatment plan goals are achieved
